Health Career Convention


Please read the following information carefully:

     Dear Student:

The following application must be completed and submitted online. Please answer the following questions and make sure all information is correct before submission. Once complete, please print out two copies of your receipt. One copy will be for your personal records, and the other will be mailed in, with your payment, to the address provided. If you would like the top portion of your receipt mailed back to you, please enclose a stamped and addressed envelope along with your payment. If you are attending with a school, your lead chaperone will receive a list of confirmed students.

I have a signed parent permission form on file at my school of attendance releasing me to attend the Health Career Convention on February 7th and 8th, 2009.
