Health Career Convention


Please read the following information carefully:

     Dear Chaperone:

The following application must be completed and submitted online. Please answer the following questions and make sure all information is correct before submission. Once complete, please print out two copies of the your receipt. One will be for you to keep for your personal records, and the other will be mailed in, with your payment, to the address provided. If you would like the top portion of your receipt mailed back to you, please enclose a stamped and addressed envelope along with your payment. If you are attending with a school, your lead chaperone will receive a list of confirmed guests.

I will enforce the following statement that my students have agreed, “It is a privilege to attend the Health Career Convention and I will behave in a professional and respectful manner to both speakers and attendees alike."
I understand that it is my responsibility to have a signed parent permission form on file for all students who will be attending the 'Health Career Convention' on February 7th and 8th. You may use an official school waiver form, or one can be printed from the HCC website.
